- New Patient Forms (if you have downloaded and completed ahead of time)

 

- Insurance Card

 

- Drivers License

 

- Payment needed for Copay/Coinsurance

 

- Prescription or Referral from Physician (if required by Insurance company)

 

- Copy of any helpful documentation from other providers such as IEP, Testing, Medical Evaluations, or prior therapy notes
