Initial Intake Process

During the intake process we will gather general contact and insurance information to get you setup with your first appointment.  You can either download our New Patient Forms ahead of time and fill them out to bring to your first appointment or arrive at least 15 minutes early to your first appointment so you will have time to fill them out in the office.
 

Generally, due to insurance requirements a prescription from you or your child’s physician will be required.  You can have this faxed to our office at (240) 256-3612.
 

Once we have received your completed New Patient Forms, along with your physician referral or prescription, our billing specialist will assist you with understanding your insurance benefits. 

 

Your First Appointment

The first appointment will be an Evaluation of your therapy needs and then an individualized treament plan will be developed to best meet your goals.
